Primary Duties:

Coordinate an array of in-home services for adults with disability to help maintain their independence. We are seeking a talented and highly motivated individual who has a sincere desire to assist people.
As a Services Coordinator you will determine the eligibility of applicants for the Nebraska Medicaid Waiver Program for Adults with Disability, conduct a needs assessment, and in conjunction with Consumers develop a plan of services. You would assist to recruit independent in home providers, negotiate rates, authorize, and monitor services. This is not an exhaustive description of job duties.


In order to be considered for this position, you must have a Baccalaureate degree in Human Services and 2 years experience in gerontology, rehab, or community health fields. Services Coordination will be provided in Consumers' homes, so travel throughout the League's Norfolk Service area is required. Applicants must also be proficient in the use of computers and various computer software programs. You will be expected to provide your own transportation, and we will reimburse you on a per mile basis. Qualified individuals with disability are strongly encouraged to apply.
